‘My Eyes Were So Puffy I Couldn’t Even Open Them’

Onemanda Preisinger experiences worry about her child’s upcoming 13th birthday celebration. Not for the usual reasons associated with a house full of clamorous preteen children, but since it’s the initial occasion she will reveal her new face to friends and relatives. “Obviously I’m going to tell all guests as they arrive, ‘Just so you know, this is not how I appear,’” states the thirty-year-old property agent from Southern Florida.

Her appearance is, well, a somewhat surprising – her face swollen and unnaturally tightened, as though secured by industrial-grade tape. Her new – and she’s eager to emphasize, short-term – appearance is the result of half a dozen aesthetic surgeries, such as an endoscopic mid-facelift, conducted by a doctor in Istanbul, Turkey, the previous month. “My poor husband teared up when he viewed me for the first time because I wasn’t able to even unseal my eyes. That indicates puffy I was,” she explains to me via online call from her house. “I needed to inform him: ‘Honey, I’m okay, I’m not in pain. I just appear as if someone jumped me.’”

Growing Trend of Youthful Facial Lifts

Based on cosmetic specialists, Preisinger is among a growing number of people choosing to undergo a rhytidectomy in their twenties and thirties – significantly before a decade before most doctors’ usual candidate age of forty to sixty. (Though many specialists are keen to emphasise the industry edict of: “We treat heredity, not years.”) “I have individuals in their late twenties requesting facelifts,” states based in London aesthetic plastic surgeon Georgios Orfaniotis, a former NHS consultant who specialises in the facial area. “It’s a major surgery, and I’m a somewhat worried when I see people choosing it as a lifestyle choice.”

Understanding the Facelift Procedure

A facelift, also known as a facelift, elevates the ligaments in the face that begin to droop as we grow older. “Our faces are structured a bit like onion layers,” says Kent-based facial plastic surgeon Marc Pacifico. “Dermis and adipose on the top, then a layer of connective tissue called the superficial musculoaponeurotic system. Think of the SMAS as the structural foundation of the face. And that is what ages us; that’s what becomes lax and pulls the dermis down with it.”

You endanger operating on individuals that have underlying problems. It’s not a practice for an ethical plastic surgeon to pursue

Once the preserve of affluent seniors, overuse of injectables – when excessive use of dermal fillers stretches the face and leads to laxity in the dermis – combined with the common adoption of slimming medications, cut-price surgical travel, and the advancement of novel, famous-figure-promoted surgical techniques are attracting a new kind of customer towards this major operation. Statistics indicate an 8% increase in facelifts over the last year in the United Kingdom; while a study found that the percentage of youthful candidates is increasing, with 32% of facelifts now performed on patients aged thirty-five to fifty-five.

“Patients are asking now to appear as the optimal form of themselves and obviously the techniques are evolving,” comments Orfaniotis. Currently, the operation du jour is the comprehensive facial lift, a technique promoted by figures including Kardashian matriarch Kris Jenner, to fashion designer Marc Jacobs.

Deep Plane Technique

While a conventional rhytidectomy involves lifting the SMAS, the deep plane facelift loosens the underlying structures beneath it, enabling doctors to restructure the face by repositioning the deeper tissue, and preventing the tight, stretched look occasionally caused by older methods. “We avoid placing stress on the dermis because we’re going below, to the deeper tissue,” explains prestigious London plastic surgeon Rajiv Grover, who specialises in the advanced method. “It is the deeper tissue that is adjusted. The skin follows along as a secondary element.” Elevating the entire soft tissue layer as a single unit allows for a more natural-looking and more durable result. It additionally implies that the procedure is not just being used for the primary goal of youth preservation – or revitalization, as the aesthetics industry describes it – but for “beautification” and contouring, too. “We’re getting many inquiries from patients aged late twenties to mid-thirties for this reason,” notes Grover.

Personal Journey

Preisinger had never planned on undergoing a facelift – at least not at the age of 30. But years of using hyaluronic acid dermal fillers in an effort at “symmetry correction” meant that by the time she reached her late twenties, she was “damaged”. “I didn’t do this because I desired to appear youthful,” she says. “I did it because the injectable ruined my face. I wish I had never using it. If I had avoided the injectable, I don’t believe I would be in this situation currently.”

Whether injectable substances ever fully dissolve has long been a debated topic in the aesthetics industry. “Research have indicated that not only do they rarely fully dissipate,” says Grover, “but they move and can obstruct drainage pathways. A contributing factor to what we term ‘puffy appearance’ is also the reality that in a sense, the face gets slightly waterlogged because its capacity to remove bodily fluid has been reduced.” Though research into the area is preliminary, warnings on dermal fillers’ potential impact on the body’s drainage have been released, and additional studies initiated. One highly regarded facial plastic surgeon, speaking on the condition of anonymity, mentioned he would avoid using injectables in a client because of the dangers they pose. “Many doctors avoid talk about this, because the companies who make filler can be quite forceful.” He’s been told accounts, he says, of plastic surgeons being landed with court orders after raising their concerns.

Choice and Surgery

For Preisinger, after beginning injecting filler, she believed she had to keep adding more – especially as it started to migrate to other parts of her face. She was just 28 when a nearby cosmetic doctor in the state proposed that a facial and neck surgery might be what she needed to finally step off the cycle of treatments. Two years later, she ended up selecting from a list of recommended hotels in the city, sent to her by the patient coordinator at her doctor’s clinic.

Originally, Preisinger had scheduled an eyelid surgery – commonly called blepharoplasty – but her doctor advised her that a facelift was the correct option for the looseness she was finding in her face. She arranged a brow lift, a cheek lift and a neck lift. The eve of her surgery, 24 hours post she’d arrived alone in Turkey, she opted to include a mouth enhancement. “Then he stated, ‘We will elevate the under-eye bags.’ He added, ‘I believe if we remove some cheek fat, it will contour your face.’ So I ended up including additional procedures,” she recalls. She paid $22,000 (£16,500) for the six-hour operation, plus a three-day|
